Hello Parents and Friends of Ludeman Center,

The State of Illinois has informed all State Operated Developmental Centers (SODCs), which includes the Ludeman Center, regarding the TWO separate consent forms Parents/Guardians will receive in the US Mail for the COVID-19 vaccine to be administered to the Ludeman Residents.

One form is for the Pfizer vaccine and the other form is for the Moderna vaccine. Because there is a possibility that both manufacturers vaccine will be available, a separate form is needed for each vaccine manufacturer.

Both vaccines have an effective rate of 94-95% efficacy. Your loved one will be administered the vaccine in two doses from the same company when the vaccine and your completed consent forms are returned.

Because there is a possibility that both the Pfizer and Moderna vaccines could be delivered to the Ludeman Center, TWO separate forms are necessary because at this time, it is not known which vaccine will be readily available at the Ludeman Center for use. Please fill out BOTH vaccine authorization forms for your loved one that resides at the Ludeman Center and return both of your completed consent forms as soon as possible.

The focus of the call is to inform and update parents and guardians with information regarding the ELDC directives and activities. ELDC Facetime and Zoom Video Chats Notification

Many Parents and Guardians have been taking advantage of the ELDC iPads to have Facetime and Zoom video chats with your loved one. Beginning December 17th through December 28th, Please contact your Residential Unit Director to set up an appointment. Available video chat times are Monday thru Friday between 9:30 a.m. and 4:00 p.m. due to the limited availability of authorized staff to ‘check out’ the iPad for use.

Hello Parents and Friends,

The Elizabeth Ludeman Center Administration has informed us that there is once again a COVID-19 Outbreak at the Ludeman Center. Ludeman Center is moved to Outbreak Status when a combined 20+ individuals (staff, supporting agency staff and residents) have tested positive for COVID-19. At this point Staff, Supportive Agency Staff, and Residents have all been impacted by the virus outbreak.

Outbreak Status Impact on Visits with Residents

Covid-19 Outbreak Status necessarily impacts visitation with your loved one. All in-person visits at Ludeman Center are currently canceled and on hold.

The 3 Visitation Phases at Elizabeth Ludeman Development Center (ELDC)

  • Phase 1 implemented when One or More ELDC COVID-19 Positive Tests, regardless of the source listed above.
  • Phase 2 – To move from Phase 1 to Phase 2, ELDC must have 14 Consecutive Days without ANY NEW POSITIVE COVID-19 CASES.
  • Phase 3 – To move from Phase 1 to Phase 3, ELDC must have 28 Consecutive Days without ANY NEW POSITIVE COVID-19 CASES.

Finally, Home Visits are also not allowed at this time. When Home visits become available, ELDC will notify all parents and guardians.

The Elizabeth Ludeman Center, Under Guidance from the Department of Human Services and IDPH has clarified the directive for visiting with your loved one that resides at the Ludeman Center.

ELDC – Visitation and Home Visit Guidance 10.6.2020

The above document has been sent to all parents/guardians. The document explains the rules and regulations for Phase 1 and Phase 3 Visits with your loved one.

  1. The current Ludeman Center Visitor status is PHASE 1.
    1. Phase 1 includes In-Person at Ludeman Visits and Video Chat Visits. Please refer to Page 4 for details.
  2. The Ludeman Center Visitor status for PHASE 3 is NOT IN EFFECT.
    1. Phase 3 includes Home Visits as well as the Phase 1 Visits. Please refer to Page 5 for details.
  3. If COVID-19 positive tests results at the Ludeman Center (staff and residents combined) are 20 or more, only Video Chat visits will be available.
